Step 2:  Complete the information. Please include your name, address, daytime telephone number, your anticipated wedding date, the number of expected guests, and the wedding services you would like included in your wedding package.
Step 3:  Call us or fax this form for a price quote.  Call our toll-free number at 1-800-729-4365.  You may also complete this form and then fax it to us at 1-865-453-0537.  Our business office hours are weekdays, 9 am to 6 pm., and Saturday until 4:30 p.m. Weddings are performed 7 days a week, by appointment only, excluding Easter, Thanksgiving and Christmas.  Thank you!

NOTE:   If you aren't able to visit us in person, we offer a video tour of the exterior and interior of the chapel, our grounds and the surrounding mountain views from the chapel.   There is a $6.00 fee for postage & handling. Indicate your interest in the video by a check mark:  Yes____.
